Parent Form Filler - User Guide
What is Parent Form Filler?
Parent Form Filler is a browser extension (and future mobile app) that automatically fills out forms for you using information you’ve saved about your family. Instead of typing the same information over and over again on school forms, doctor visits, sports registrations, and other paperwork, this tool does it instantly.
The problem it solves: As a parent, you probably fill out dozens of forms, regularly, with the same basic information about your family. This gets tedious and time-consuming, especially when you’re dealing with multiple children and different activities.
How it helps: Store your family’s information once, then automatically fill any form with just a few clicks. It works on websites and PDF documents, and it’s smart enough to figure out which information goes in which field.
What You Need to Get Started
- Chrome browser or Firefox (coming soon for Safari, the Apple Mac Default, and iphone and Android)
- 5-10 minutes to set up your family profiles
Installation & Setup
Step 1: Install the Extension & Sign Up
- Download and install the Parent Form Filler extension (visit here)
- Look for the new extension icon in your browser toolbar.
- Click the icon to start setup.
- You will be prompted to sign up for an account. This is required to access the AI functions that power the smart form-filling capabilities.
Step 2: Set Up Security
Choose your security level:
- Quick Setup: Easy access, no master password needed.
- Secure Setup: Extra protection with a master password (recommended).
If you choose Secure Setup, create a strong master password that you’ll remember. This encrypts all your family’s information.
Step 3: Choose Your Location
Select your geographic region (USA, Canada, UK, Australia, etc.). This helps the extension recognize form fields that are formatted differently in different countries.
Step 4: Add Your Family Information
For each family member, you can add:
- Basic Info: Full name, date of birth, gender
- Contact: Phone numbers, email address, home address
- Medical: Doctor information, allergies, medications, insurance
- Emergency Contacts: Names and phone numbers
- Other: Employment details (for adults), school information
Tip: You don’t have to fill in everything at once. Add what you need and come back later for more.

Filling PDF Documents
- Open a PDF form in your browser.

- Click the extension icon - it automatically detects it’s a PDF.
- Select the family member.
- Click “Analyze PDF.”
- Review and fill the same way as web forms.
- Download or print the completed PDF.
Understanding Field Matching
The extension is smart about matching your stored information to form fields, even when they have different names. For example:
- “First Name” and “Child’s First Name” both get filled with the same information.
- It shows you a confidence percentage—how sure it is about each match.
- You can manually adjust any field that doesn’t look right.

- Go to settings or “Edit Data.”
- Add a new profile.

- Save the profile.

- Try downloading the PDF first, then opening it.
Forgot Your Master Password
